While a sincere attempt is made to adhere to the schedule as much as possible, emergencies will be seen and worked in with the regular appointments as necessary. If an emergency causes a serious delay, we will try to notify you before you arrive at the office.

You will be asked to complete a brief medical history which requires you to list medications you are currently taking. Please bring your glasses, medical prescriptions, Medicare card, Medicaid card and/or any current insurance information. If you have been referred to our office by another physician or one of our patients, please inform us at the time of your initial visit. If you participate in an HMO or PPO plan, you need a referral letter from your primary care physician before the plan pays.

The eye examination may include dilating the pupil. If both eyes are dilated, we recommend that you have someone drive you home. If you have sunglasses, please bring them with you.

Returning patients should notify the receptionist of address or phone number changes that have occurred since their last visit.

We do everything we can to limit the cost of eye care. Charges for office visits and surgery may vary with the complexity of your problem, the time required to treat your problem, and the types of diagnostic procedures performed during your visit. If you have questions about our fees, please ask us to explain them.

So it is important for you to know that Minadeo Eye Center accepts Medicare assignment to make cataract care affordable for seniors. For those who use our Out-Patient Cataract Surgery Program, the Minadeo Eye Center will accept the Medical allowable fee for a covered service and the anticipated Medicare payment of 80% of that allowable. The 20% difference may be covered by supplemental insurance or may be discounted in cases of severe hardship. Other financial arrangements are available.

Patients with private insurance will be asked to use the insurance copy provided with their receipt to file with their insurance carrier. The receipt you receive after each office visit is in fact a statement containing all the information required from the doctor.

If surgery is indicated, it is the patient's responsibility to furnish the office with the necessary insurance forms, properly filled out and signed, prior to the date of surgery. Our office will file for surgical procedures.

Although the office will wait a reasonable time for amounts covered by private insurance on surgical procedures, the patient is ultimately responsible to see that the bill is paid. See us for other financial services.
